A Higher-Complex Carbohydrate vs Conventional Low-Carbohydrate Diet in Gestational Diabetes Mellitus results in Lower Postprandial Free Fatty Acids: A Randomized Controlled Trial.

/interpretation CHOICE resulted in 20% less postprandial FFA exposure and blunting of the expected rise in TG/glycerol, which may mitigate maternal IR and fetal fat accretion. Glycemic control was clinically acceptable despite a 12% greater post-breakfast glucose exposure. Thus, lower dietary fat and higher quality carbohydrates produced a synergistic attenuation of TG/FFA with effective glycemic control in GDM. Clinicaltrials Gov registry NCT02244814,

Bidirectional longitudinal associations between adiposity and emotional and uncontrolled eating in an observational UK population-based cohort.

Findings challenge assumptions that UE and EE are drivers of adult weight gain and instead suggest that higher adiposity may contribute to the development or escalation of dysregulated eating behaviours. Interventions may benefit from addressing the behavioural and psychological consequences of higher body weight alongside traditional weight-focused strategies.

Childhood plant-based diet quality and cardiometabolic health, metabolite profiles, and dietary costs: A population-based cohort study.

Gradual shifts toward healthy plant-based diets may support children's cardiometabolic health without clear evidence of changes in dietary costs, whereas unhealthy plant-based diets tended to have lower dietary costs but less favorable metabolic profiles.

Estimating the effects of Nordic diets on the risk of major adverse liver outcomes: a target trial emulation across two cohorts in Sweden.

Under the strong assumptions of no unmeasured confounding, selection bias or measurement error, we estimated no risk reductions from a population-adapted HND or a NNR23 diet on the 24-year risk of MALO when compared to each other or no intervention. However, for all-cause mortality, we estimated meaningful risk reductions following the HND.

Nutrition security screening in clinical care: a feasibility and implementation evaluation in family medicine and subspecialty clinics.

Integrating NI screening into routine clinical care was feasible and acceptable, with strong uptake through the patient portal. Clinical correlates and reported barriers highlight the utility of NI assessment, complementary to FI screening, for informing tailored nutritional and social interventions.

Peanuts-derived plasma lipids and lower risk of type-2 diabetes: Association mediated by body mass index. An intervention and case-cohort study.

These results highlight peanuts as a source of dietary fats that may be linked to lower risk of T2D. Defining adipose tissue profiles in children with Crohn's disease.

This study describes a specific adiposity pattern in children with Crohn's disease; 'Thin on Outside Fat on the Inside Crohn's associated Obesity' (TOFICO). This is characterized by increased visceral and subcutaneous adipose tissue in the context of normal liver fat and BMI. We have shown that visceral obesity is a robust marker of bowel inflammation in Crohn's disease and is not affected by anthropometric factors, unlike BMI.

Bridging preclinical and clinical efficacy of SIK2/SIK3 inhibition in inflammatory bowel disease through integrated murine, patient-derived models, and phase 2a trial-based analyses.

Integrated analyses across murine, patient-derived, and clinical trial datasets demonstrate conserved immune-epithelial responses to SIK2/SIK3 inhibition by GLPG3970. These findings support the value of patient-relevant experimental platforms to assess target engagement and therapeutic activity prior to clinical testing, strengthening translational alignment in IBD drug development.

Isolated perianal fistulizing Crohn's disease in contemporary clinical practice: a multicenter ECCO-CONFER cohort study.

Approximately one-third of patients with IPFCD developed luminal Crohn's disease over long-term follow-up, typically after several years. However, many patients remain free of intestinal involvement, reflecting a heterogenous disease course and highlighting the need for standardized evaluation and improved risk stratification.

Artificial intelligence in liver transplant allocation: Promise, pitfalls, and ethical safeguards.

We suggest a framework for responsibly integrating AI into liver allocation focused on fairness monitoring, transparency, and explainability, human oversight, and multi-stakeholder governance. When applied properly, AI can potentially enhance both utility and equity in liver transplantation-but only if its use is guided by clear ethical principles and ongoing evaluation in real-world clinical settings.

Biliary complications in pediatric living donor liver transplantation: An international multicenter analysis of risk and prognostic factors.

Ultimately, bile leak and anastomotic stricture remain significant threats to graft survival. The identified risk factors offer insights for risk stratification and prevention strategies aimed at improving outcomes in children undergoing LDLT.

Evaluation of available scoring systems in liver transplantation for severe acute-on-chronic liver failure (ACLF).

Both TAM (OR 1.91, p=0.0001) and SALT-M (OR 1.09, p 25) may help identify a small subset of patients with markedly poor post-LT outcomes. However, given their modest performance, these scores should support, not replace, multidisciplinary decision-making and their applicability should be evaluated in a prospective fashion.

PD brushing for BC and FISH is generally safe with modest diagnostic accuracy for malignancy. While novel diagnostic tests such as molecular biomarkers in pancreatic fluid for detecting PC are still needed, PD brushings can assist in the detection of PC with low rates of PEP in select subjects.

The Effect of Pancreatic Exocrine Insufficiency and Pancreatic Enzyme Replacement Therapy on Gut Microbiome Composition in Pancreatic Disease: A Prospective Cohort Study.

Treatment with PERT leads to significant changes in the gut microbiome composition of patients with pancreatic disease. Changes include a significant reduction in potentially pathogenic bacteria and so may contribute to the survival benefits seen with PERT treatment in pancreatic disease.

These findings support a cellular ecosystem framework for understanding therapeutic response and resistance in inflammatory bowel disease. Translating this framework into precision care will require functional validation of candidate mechanisms and prospective clinical evaluation of predictive tools for treatment selection.
