One-carbon metabolism nutrients and polygenic risk scores in relation to breast cancer risk: A nested case-control study within the EPIC cohort.

Higher intake of B-vitamins was associated with reduced BC risk. Adipose tissue-specific downregulation of 1CM genes may differentially influence BC risk by menopausal status. These findings support the biological plausibility of 1CM in breast carcinogenesis and highlight the potential importance of adequate B-vitamin intake.

Sugar rationing during the first 1000 days and Early-Onset Cancer: A Natural Experiment.

Restricting sugar intake during the first 1000 days is associated with a reduced risk of early-onset cancer, extending the disease-free lifespan. The divergence between reduced incidence and unchanged mortality suggests early-life metabolic environments primarily influence tumor latency rather than biological aggressiveness. These findings highlight the potential long-term public health implications of early-life dietary guidelines against the rising burden of early-onset cancer.

The Validity of Measuring Energy and Nutrient Intake with the Remote Food Photography Method (RFPM) in Adolescents.

The RFPM can accurately estimate the meal-level EI of adolescents in controlled settings; however, significant underestimation occurred in free-living settings in both studies. Previous studies demonstrated the inability to accurately assess adolescents' free-living intake, and the current results extend these findings to the RFPM. Risk of serious infection, myocardial infarction, stroke, and thromboembolic events in patients in the United States with ulcerative colitis treated with tofacitinib compared with biologic treatments.

No significant risk differences were observed among patients with UC initiating tofacitinib compared with biologics in a large US claims database. The hostile abdomen dilemma: the safety of primary anastomosis in the presence of an intraoperative abscess in Crohn disease.

While an intraoperative abscess increases operative difficulty, it does not independently compromise anastomotic integrity. Primary anastomosis in an infected field is safe, allowing surgeons to spare appropriately selected patients at experienced centers the profound morbidity of an unnecessary stoma.

A spatially organized immune-stromal program drives the fissure-to-fistula continuum in Crohn's disease.

Fissures represent active immune-stromal niches in which coordinated remodeling programs support the development of the fistula microenvironment. These findings identify early fissure-associated remodeling as a potential therapeutic window and highlight CXCR4, complement, and immune-stromal adhesion pathways as candidate targets to intercept fistula progression in Crohn's disease.

Retention of the ileocolic pedicle correlates with higher postoperative endoscopic recurrence risk in Crohn's disease ileocolic resection.

Postoperative ER risk was positively correlated with RIAL in CD. Though these correlative data reveal at a statistical association between ileocolic pedicle division and reduced ER risk, our observational analysis does not prove a causal protective effect of pedicle resection.

Behavioral alcohol use disorder therapy in early liver transplant evaluation for alcohol-associated liver disease - a multicenter retrospective cohort study.

bAUDT is more common among patients with risk factors for return to use and with the medical and financial stability to participate in therapy. Although associated with increased waitlisting adjusted odds, bAUDT does not significantly predict return to alcohol use pre- or post-LT. Future research should explore optimal methods of integrating bAUDT into the LTE process to improve access and minimize barriers to LT.

Bilateral duct-to-duct anastomosis in dual left graft living donor liver transplantation: Bile duct position assessment after graft procurement and perfusion - a retrospective cohort study.

Bilateral D-D is therefore achievable when guided by intraoperative assessment of the bile duct-portal vein relationship after composite rotation: hepaticojejunostomy should be performed without hesitation in unfavorable configurations, and intraoperative portal vein stenting considered selectively when narrowing is detected. In carefully selected patients, bilateral D-D is a feasible strategy that preserves endoscopic access; these single-center observations are exploratory and warrant prospective validation.

The rejectome of acute t-cell mediated rejection in clinical liver transplantation.

Plasma proteomics and predictive modeling identified a tissue correlative panel of 11 biomarkers linked to immune function and liver metabolism that differentiates TCMR from no rejection and proteomic profiles in other liver diseases. These data highlight multi-tissue proteomics features of the alloimmune microenvironment during clinical TCMR and suggest that a clinical plasma proteomic 'liquid biopsy' can be developed for management of TCMR in LT.

Clinical characteristics and therapeutic implications of MTAP deletion in pancreatic cancer: A nationwide genomic analysis.

MTAP deletion is a frequent genomic alteration in pancreatic cancer. Although it was not an independent prognostic or chemotherapy-predictive biomarker, our comprehensive clinicogenomic findings provide a foundation for future evaluation of MTAP-focused therapeutic strategies in pancreatic cancer.

The degree of obesity correlates with the severity of acute pancreatitis: An international multicenter cohort analysis of 2244 patients.

Obesity dose-dependently elevates the odds of pre-existing comorbidities and complications. Gradual weight loss may also have a positive effect and the impact of patient education should be investigated.

Trypsinogen-activated macrophage exosomes enriched in Rap1a exacerbate inflammation and predict severity in acute pancreatitis.

Trypsinogen-activated macrophages contribute to systemic inflammation in AP via the release of Rap1a-enriched exosomes. Plasma exosomal Rap1a demonstrates potential clinical utility as an early, mechanistically-grounded biomarker for predicting SAP severity.
