| Effects of exercise timing and metabolic state on post-exercise energy intake and appetite - A randomized crossover trial.
Evening fasted exercise induces pronounced compensatory eating and elevated 24 h appetite, despite greater fat oxidation. Pre-exercise feeding alters substrate use but does not reduce intake, underscoring a dissociation between metabolic and behavioral regulation and the primacy of timing in post-exercise appetite control. Clinical trial registry (NCT06947824; |
| Metabolites Co-correlating with Maternal Obesity and Gestational Weight Gain and Offspring Childhood Obesity: A Prospective Birth Cohort Study.
Metabolomic profiles associated with and may partly mediate the association between maternal obesity, particularly ppBMI, and increased risk of COWO. These findings warrant validation in independent cohorts. |
| Source- and solubility-specific choline, gut microbiota, and dyslipidemia risk: TMAO-associated and non-TMAO-associated patterns in a prospective cohort study.
Dietary choline source and solubility differentially associated with dyslipidemia risk through TMAO-associated and non-TMAO-associated patterns, with gut microbiota as key modulators. |
| Trajectory and predictors of human milk intake from 1 to 8.5 months of lactation: Findings from the multicenter MILQ cohort study.
Maternal BMI, infant sex, EBF for 6 months, WAZ, and breastfeeding frequency predict HM intake between 1-8.5 months of lactation, while EBF, WAZ, and breastfeeding frequency shape the HM intake trajectory. Clinical trial registry number Registered at clinicaltrials.gov (NCT03254329). URL |

| Appendicectomy in ulcerative colitis: a narrative review of epidemiology, pathophysiology, and emerging therapeutic implications.
While medical management remains the first line, appendicectomy may act as an adjunct surgical option in carefully selected cases before escalating to a colectomy. Large multicenter studies and randomized clinical trials are required to identify strategies for patient selection, potential complications, durability of response, and long-term outcomes. |
| Artificial intelligence assessment of endoscopic severity and extent: A machine learning approach to continuous evaluation of endoscopic inflammation in ulcerative colitis.
AI-ESe enabled detailed assessment of endoscopic inflammation, capturing heterogeneous burden of disease within conventional endoscopic severity classifications in ulcerative colitis. Granular disease assessments using AI-ESe demonstrate an enhanced approach to disease evaluation. |
| Classifications of rheumatological extraintestinal manifestations in inflammatory bowel disease: Unmet needs in clinical practice and trials.
Evidence from this review reflects considerable methodological variability in the assessment of rheumatological EIMs in IBD. Standardized definitions, harmonized outcome measures, and the development of EIM-specific PROs are needed to support consistent endpoint selection and improve the evaluation of rheumatological outcomes in future clinical trials. |
| Increased risk of respiratory syncytial virus infection in patients with active inflammatory bowel disease: a matched cohort study.
Patients with active IBD had higher risk of RSV infection. Increased RSV risk was observed both among patients with active IBD with and without systemic corticosteroid exposure. Among RSV-vaccinated patients, RSV infection rates were not significantly different between active and nonactive IBD. Further prospective studies are needed to validate these findings. |
| The association between artificial light at night and inflammatory bowel disease incidence, surgery, and health services utilization: population-based observational studies.
The association between ALAN and IBD is heterogeneous. Additional research is needed to understand how ALAN impacts IBD and identify other environmental exposures contributing to IBD etiology. |
| Trained immunity: An unexplored component of IBD pathogenesis.
From a translational perspective, we consider how TI may favor relapse even during clinical remission and discuss its potential relevance for patient stratification, biomarker identification, and the development of novel therapeutic strategies. Ultimately, integration of TI into IBD pathophysiology may corroborate more lasting, mechanism-based approaches to remission. |
| Underevaluation of bowel urgency in clinical care: patient experiences and outcomes in ulcerative colitis: a retrospective cohort study.
Urgency is inconsistently assessed in routine clinical care, although when present it provides additive information to other measures of disease activity in UC and is associated with an increased risk of disease-related events. |
| Upadacitinib as a rescue therapy for steroid- and infliximab-refractory acute severe ulcerative colitis: short- and long-term effectiveness and safety in a multicenter cohort study.
In this multicenter cohort of patients with ASUC who failed IV-CS and IFX rescue, third-line UPA was associated with a low colectomy rate and sustained response up to 1 year, with an acceptable safety profile. UPA may provide sufficient disease control to prevent or delay colectomy, or to serve as a bridge to alternative therapies. Summary In this retrospective multicenter cohort (11 centers), 50 patients with severe ulcerative colitis refractory to intravenous corticosteroids and infliximab received upadacitinib. Improved symptoms and C-reactive protein (CRP) levels occurred by week 1 and persisted to week 48; colectomy occurred in 22% and adverse events in 24% of patients. |

| Comparative performance of intestinal ultrasound activity indices in pediatric ulcerative colitis: a prospective study.
MUC, UC-IUS, and Civitelli demonstrated comparable accuracy for detecting moderate-to-severe endoscopic activity in pediatric UC. BWT alone performed similarly to composite indices. Combining IUS with FC or PUCAI may provide modest additional diagnostic value, supporting a multimodal, noninvasive approach for disease monitoring and treat-to-target strategies. |
| Corticosteroid-sparing effects of risankizumab versus ustekinumab in patients with moderately to severely active Crohn's disease: post hoc results from the phase 3b SEQUENCE trial.
In patients with CD refractory to anti-TNF therapy, higher rates of corticosteroid-free clinical, endoscopic, and quality of life outcomes were achieved with RZB versus UST. Both treatments were well-tolerated. Clinical trial registration |
| Methotrexate for inflammatory bowel disease: the MICI-METHO multicenter retrospective cohort study.
The French multicenter MICI-METHO study is the first to provide a comprehensive overview of the real-world use, persistence, and safety of methotrexate in patients with IBD. |

| Continuous normothermic machine perfusion is independently associated with lower risk of acute kidney injury and renal replacement therapy in donation after circulatory death liver transplantation: a propensity score matched analysis.
After weighting for multiple factors, NMP-DCD was associated with lower odds of AKI at 48-hours (OR:0.65, CI: 0.53-0.81, p=0.001), stage 2 and stage 3 AKI at 48-hours (OR: 0.60, CI: 0.42-0.85, p=0.02) and RRT at day 7 (OR:0.31, CI:0.13-0.65, p=0.003) compared to SCS-DCD. In conclusion, compared to SCS-DCD, NMP-DCD was independently associated with lower odds of AKI at 48-hours, of stage 2 and stage 3 AKI at 48-hours from LT and of RRT requirement at day 7 from LT, an effect that extended for the first 3 post-LT weeks. |
| Efficacy of different portal vein reconstruction techniques in liver allografts with multiple portal veins in living donor liver transplantation: A propensity score matched analysis.
Propensity score match analysis with single PV recipients revealed significantly higher early allograft dysfunction (23.8% vs 10.9%, p=0.02) and biliary leak (14.9% vs 5%, p=0.01) in this cohort, however vascular complications and survival rates were comparable. Overall, the findings suggest that when reconstruction is tailored to the individual vascular anatomy, al three techniques are safe, effective, and feasible, enabling broader safe utilization of donors with PV variations. |
| Liver parenchymal quality stratifies outcomes after hepatic resection for hepatocellular carcinoma.
Histologically normal liver identifies a distinct HR subgroup with favorable oncologic outcomes. The conventional cirrhotic/non-cirrhotic dichotomy underestimates the prognostic impact of underlying parenchymal disease. |

| Outcomes of Post-Endoscopic Ultrasound Pancreatic Cancer Compared to Detected Pancreatic Cancer: A Multicenter Cohort Study.
Nearly 5.4% pancreatic cancer occur after negative EUS in the previous 6-18 months. D-PC had higher chance of surgical resection and chemotherapy but also higher rates of mortality at 3-years. Further large scale studies with patient and procedure level data assessment to allow root-cause analysis is needed to determine its etiology and associated factors to limit its incidence. |

| Global, regional, and national burdens of pancreatic cancer and pancreatitis among older adults from 1990 to 2021: A comparative investigation.
Pancreatic cancer and pancreatitis show divergent global trajectories in older adults, driven by socioeconomic development, health system access, and modifiable risk factors. The rising pancreatic cancer burden requires strengthened prevention and early detection, especially in low-resource settings. Public health efforts should prioritize smoking cessation, glycemic control, and reduced alcohol use to mitigate disease impact in aging populations. |
| Spectrum and determinants of acute and chronic pancreatitis in choledochal cyst.
Apart from a higher prevalence of pancreas divisum, no predictive factors were found for CP. This should be interpreted cautiously because of retrospective non standardized imaging assessment. |
