IBD is associated with CI and increased dementia risk, with biologics potentially mitigating neuroinflammation-related decline. More longitudinal studies and randomized clinical trials, also on intermediate endpoints, are needed to clarify the neuroprotective role of some therapies and optimize treatment strategies.

Best (but oft forgotten) practices: Applying the target trial framework in nutrition research.

Protocol components, including eligibility criteria, treatment strategies and assignment procedure, outcome and follow-up, causal contrast and statistical analysis plan, are clearly specified and subsequently implemented using available observational data. In this article, we explain the target trial framework approach, describe examples from nutrition research, demonstrate the practical steps of performing a target trial study, and discuss the methodological challenges and opportunities of applying the framework to nutrition research.

Individual glycemic responses scale predictably with the glycemic index: evidence against person-specific food effects.

In healthy adults under standardized conditions, inter-individual variation in glycemic responses is predominantly accounted for by variability in day-to-day glucose tolerance, propagating through the GI ratio. The GI concept performs within the reproducibility limits of input data.

Matrix metalloproteinases in intestinal fibrosis and fibrostenosing Crohn's disease: Current evidence, mechanistic hypotheses, and translational challenges.

Candidate biomarkers and emerging therapeutic strategies remain investigational, and no MMP-directed approach has yet demonstrated clinical utility for routine fibrosis staging or stricture prevention. Future research directions should prioritize longitudinal human studies, mechanistic elucidation of how MMPs influence fibrosis and/or smooth muscle hyperplasia, improved linkage between tissue biology and imaging, fibrosis-specific endpoints, and rigorous validation before clinical trial translation can be justified.

Transperineal contrast-enhanced ultrasound: a rapid and reliable tool for differentiating abscesses from inflammatory masses in perianal Crohn's disease.

Transperineal CEUS significantly improves inter- and intra-observer agreement and diagnostic confidence compared with conventional B-mode TPUS when differentiating perianal abscesses from inflammatory masses/phlegmons in CD and frequently leads to lesion reclassification in clinically ambiguous cases.

Future opportunities lie in incorporating underutilized data domains, such as psychosocial factors, -omics, and intraoperative physiologic streams, and leveraging GenAI to enhance clinical decision support, research scalability, and patient-clinician communication. AI is poised not only to refine prediction, but to reshape the conceptual and operational landscape of liver transplantation.

Frailty 1 year after liver transplant is associated with long-term mortality: From the Functional Assessment in Liver Transplantation (FrAILT) Study.

Patients who transitioned from not frail pre-LT to frail 1y post-LT had a 2.5-fold increased risk of post-LT mortality (95%CI 1.3-4.8, p=0.005) compared to patients who remained not frail. These findings suggest the 1y period after LT may be a critical window for evaluating whether targeted frailty-mitigation strategies may improving long-term survival among LT recipients.

The liver frailty index improves mortality risk prediction for GEMA-Na.

While GEMA-Na reduces gender disparities related to renal dysfunction, the added prognostic value of frailty highlights muscle health as an independent and actionable determinant of outcomes. These findings support routine frailty assessment to better stratify risk and to potentially guide timely interventions such as prehabilitation or expedited LT.

Clinical Variables Associated With Islet Yield and Long-Term Metabolic Outcomes following Total Pancreatectomy With Islet Autotransplantation.

Islet yield is a key determinant of long-term metabolic outcomes following TPIAT. Although the primary indication for surgery remains pain relief, patient factors including age, BMI, prior pancreatic surgery, and duration of symptoms influence islet yield and can help inform expectations for postoperative glycaemic outcomes. Notably, shorter symptom duration was associated with higher islet yield, supporting timely evaluation and referral for TPIAT before further pancreatic interventions that may compromise islet recovery.

Comparative Analysis of Outcomes and Readmissions Trends in Adult and Pediatric Acute Biliary Pancreatitis: A Nationwide Analysis.

Despite higher comorbidities in adults, ABP-specific local and systemic complications and mortality were similar between adults and pediatric patients. Same-admission cholecystectomy (CCY) was associated with a significant reduction in 30-day readmissions in both groups, highlighting its importance as a key intervention across age groups in acute biliary pancreatitis (ABP) cases.

Emodin Alleviates Microcirculation Disorders in Acute Lung Injury Associated with Severe Acute Pancreatitis by Inhibiting Ferroptosis of Vascular Endothelial Cells.

Furthermore, Emodin upregulate the expression of GPX4 and SLC7A11, mitigate mitochondrial damage, and downregulate the expression of NCOA4 in HUVECs. Emodin inhibit ferroptosis in HUVECs by attenuating NCOA4-mediated iron autophagy, thereby ameliorating pulmonary microcirculation disorders in SAP-ALI rats.

Cigarette smoking drives earlier development of pancreatic adenocarcinoma among carriers of hereditary pancreatic cancer susceptibility genes.

Cigarette smoking is associated with earlier development of PDAC among PV carriers, but smoking cessation of 10 years or more mitigates this effect. If confirmed in larger studies, these findings suggest smoking history should factor into the age at which PDAC surveillance begins.

Pancreatic cyst epidemiology defined by endoscopic ultrasound: A single-center longitudinal study.

PCLs are a population health problem. Mucinous phenotypes and UCs represent most PCLs and are found in older patients. EUS has a low threshold for cyst recognition (< 2 mm) and highlights UCs as a potential precursor to IPMN-BD.

Epigenetic biomarkers in inflammatory bowel diseases-computational challenges and opportunities.

We then discuss how recent advances in computational methodology-spanning data harmonization, integrative modeling, and interpretable machine learning-can help bridge the gap between complex datasets and reliable, deployable biomarkers. Finally, we propose a forward-looking roadmap for study design and validation aimed at moving the field toward routine clinical implementation, thereby realizing the full potential of epigenetics in IBD.

Towards early detection of pancreatic cancer: The current status of cohort studies by the Diabetes-Pancreatic Ductal AdenoCarcinoma Working Group.

There are also ongoing studies on the "science of NOD" to understand its pathogenesis. Overall, the DM-PDAC Working Group has been successful in its mission to further our understanding of the link between diabetes pancreatic cancer.
