And, the DGA has far-reaching implications for non-dietary policy, which have not been fully considered yet. Until we, as nutrition professionals, can have productive conversations about the nuances related to science, messaging, and policy, a push to include sustainability in the DGA will be counterproductive.

Early immunomodulator therapy reduces the risk of intestinal resection in Crohns disease: a systematic review and meta-analysis.

Early IM therapy is associated with a substantially lower risk of intestinal resection in CD compared with later or no IM therapy. The most consistent benefit occurs when treatment is initiated within the first 1-2 years after diagnosis, supporting the concept of a therapeutic "window of opportunity" during which timely IM initiation may favorably alter the disease course.

The predictive value of intestinal ultrasound for treatment response in inflammatory bowel disease: a systematic review and pooled data analysis.

Intestinal ultrasound predicts response at an early stage after treatment initiation, both in CD as well as in UC. In anti-TNF-treated CD patients, 23% and 27% decrease in BWT after 4-8 weeks and 12-16 weeks of treatment initiation predicts outcomes.

Associations Between Extreme Temperature Events and Daily Step Counts by Weight Status and Modifying Effects of Socio-Demographic Factors: A Longitudinal Study.

These findings suggest that individuals with low-income or high BMI are associated with heightened vulnerability to heatwaves, whereas individuals with low-income or normal BMI are linked to greater sensitivity during cold spells.

Determining the half-life and turnover rate of EPA, n-3 DPA, and DHA in humans using the natural abundance of carbon-13: A secondary analysis of a randomized clinical trial.

This is the first study to investigate n-3 PUFA half-lives and turnover in humans using GC-IRMS and the natural variance of 13 C in commercial fish oils. These results were similar to preclinical model values and other clinically used methods but adds the first estimate for n-3 DPA half-life and turnover. Compound specific isotope analysis (CSIA) is a valuable tool in human studies for determining n-3 PUFA half-lives and turnover, and factors affecting them including diet, exercise, sex, genetics, and disease.

Intention-to-Treat Estimates of Prenatal Ramadan Exposure on Child Height-for-Age in the Demographic and Health Surveys.

Restricting samples to multifaith regions and accounting for growth faltering resolves conflicting findings, yielding a robust negative effect of 0.14-0.16 z-score points by age 4. Further research is needed to disentangle the specific pathways, including fasting, dietary changes, hydration, and sleep disruption, through which Ramadan exposure affects fetal development, in order to inform targeted public health guidance. Sustained effects of combined maternal and child SQ-LNS in early life on growth and developmental outcomes at preschool age: an individual participant data analysis of randomized trials in Ghana and Bangladesh.

We observed a sustained impact of maternal plus child SQ-LNS on growth and language development among female children, but not among males. Further research is needed to investigate mechanisms underlying the sex difference in response to nutrient supplementation in early life. Disentangling the gut microbiome and inflammation in inflammatory bowel diseases: longitudinal observations from the IBSEN III study.

Longitudinal TCR repertoires in ulcerative colitis patients show features distinguishing disease states.

Overall, these results highlight the importance of analyzing Vβ gene usage and K-mer enrichment in TCR repertoires, particularly given the lack of public clones across patient cohorts. Future studies characterizing the specific antigenic targets associated with these features will pave the way for biomarker discovery in UC.

Quality of healthcare delivery in inflammatory bowel disease pregnancy: a scoping review.

Given that IBD has implications for patients during pregnancy, a dedicated exploration of how quality of care may be optimized for this patient group is warranted. In this scoping review, we apply a framework assessing structural, process, and outcome indicators to examine the current quality of healthcare in IBD pregnancy, identify and discuss barriers to achieving quality care, and systematically appraise the existing models used to provide care for IBD patients during their pregnancies.

Skin-deep connections: likelihood of skin-scarring history in patients with stricturing/penetrating Crohn disease compared with patients with nonstricturing/nonpenetrating disease and non-inflammatory bowel disease controls.

Patients with stricturing/penetrating Crohn disease had significantly more hypertrophic/keloid scars than those with nonstricturing disease in this cross-sectional proof-of-concept study, suggesting potential shared fibrotic mechanisms between cutaneous and intestinal fibrosis. If corroborated in larger and more longitudinal cohorts, overcoming potential confounders such as postsurgical scars, this novel association may help identify patients at risk and facilitate development of targeted antifibrotic therapies.

Transitioning to adult care for patients with IBD: the impact of a structured IBD transition program on service engagement.

Structured transition programs in IBD can improve engagement of patients with IBD services with significantly lower rates of disengagement with IBD services seen in our study. Large prospective studies are needed in this field to further investigate this and the impact of these programs on disease outcomes.

Ustekinumab resistance in individuals with ulcerative colitis is associated with an alteration of a subset of proinflammatory mucosal regulatory T cells.

Nonresponse to ustekinumab in UC is linked to a mucosal immune environment enriched with proinflammatory T cell phenotypes and impaired regulatory T cell function. These findings suggest that Tregs are both targets and potential biomarkers of ustekinumab response, with their phenotypic and transcriptional features providing insight into mechanisms of therapeutic resistance.

Mapping adaptive immune responses toward fungal antigens in inflammatory bowel disease using T cell repertoire sequencing and phage-immunoprecipitation sequencing.

CD is associated with expansion of fungal-specific T cell clonotypes, but S. cerevisiae peptide-based PhIP-Seq reveals limited shared IgG reactivity. Broader antigen libraries and complementary assays capturing glycosylated and virulence-associated fungal epitopes will be needed to comprehensively define antifungal humoral immunity in IBD.

Donor Type, social deprivation, and long‑term outcomes in pediatric liver transplantation: A 30‑year population‑based cohort.

In this population-based cohort study, LDLT was associated with significantly better long-term patient and graft survival and a lower risk of chronic kidney disease compared to DDLT. Socioeconomic disadvantage negatively impacted outcomes primarily among DDLT recipients, highlighting the need to improve equitable access to LDLT and to strengthen targeted post-transplant support for socioeconomically vulnerable families.

Pancreatic quantitative sensory testing (P-QST) for pain assessment in Indian patients with chronic pancreatitis.

Cold pressor endurance time was shorter in CP (AIG: 54 vs 74 s, P < 0.001; AIIMS: 103 vs 114 s, P = 0.013). CPM responses were higher in CP (AIG: 5% vs 0%, P = 0.002; AIIMS: 5% vs 3%, P = 0.014) CONCLUSION: P-QST showed consistent, widespread hyperalgesia in Indian patients with painful CP, providing a window of opportunity to identify central sensitization and validating its clinical utility in assessing central pain modulation across diverse populations.

Granulomas in downstream lymphatic vasculature cause lymphocyte plugs upstream, followed by lymphangiectasia, edema, stiffening of the part and boggy mucosa.
