BEST PRACTICE ADVICE 9: Endobariatric and bariatric surgery are not advisable for patients with MetALD due to the lack of safety and efficacy data as well as concerns regarding accelerated liver disease progression with continued alcohol use after surgery.

Higher VDZ trough concentrations were associated with remission, most robustly for endoscopic remission in UC. As these associations are largely cross-sectional, they may reflect reverse causation rather than a dose-response relationship. Prospective interventional trials are required before proactive TDM can be recommended.

Endoscopic Artificial Intelligence for Invasion-Depth Prediction in Superficial Upper Gastrointestinal Cancer: A Diagnostic Test Accuracy Meta-Analysis.

Endoscopic AI shows clinically meaningful, but validation-dependent, accuracy for invasion-depth prediction, supporting stronger rule-in than rule-out utility for submucosal invasion. Its most plausible near-term role is second-reader support rather than stand-alone determination of ESD candidacy; independent patient-level external validation should be considered essential before clinical deployment. Medication Use and Risk of Microscopic Colitis: A Propensity-Matched Analysis of US Adults ≥ 50 Years (2014-2024).

In this large cohort, NSAIDs, PPIs and SSRIs were associated with an increased risk of MC, whereas no significant association was observed for ACEIs, ARBs or statins. Medication review should be considered in patients with new-onset chronic diarrhoea.

Alcohol-Use Trajectories and Major Adverse Liver Outcomes in MetALD: A Nationwide Longitudinal Cohort Study.

Alcohol use in MetALD was highly dynamic, and hepatic risk was better captured using longitudinal drinking trajectories. Both intermittent and consistent alcohol reduction were associated with lower MALO risk, with a stronger association for consistent reduction. Consistent reduction was also accompanied by favorable changes in selected cardiometabolic risk factors.

COMPARATIVE REAL-WORLD EFFECTIVENESS OF BIOLOGIC THERAPIES FOR PREVENTING POSTOPERATIVE ENDOSCOPIC RECURRENCE OF CROHN'S DISEASE.

Comparison of demographic, clinical, and psychological characteristics among patients with irritable bowel syndrome meeting the Rome clinical, Rome V, and Rome IV criteria.

The Rome clinical criteria identify a clinically meaningful IBS population, with approximately one in five individuals missed by the Rome V criteria. These patients have a disease burden comparable to those meeting the Rome V criteria only.

Long-term Outcomes After Endoscopic Resection for pMM/SM1 Esophageal Cancer: A Multicenter Prospective Cohort Study.

Janus Kinase Inhibitors for Acute Severe Ulcerative Colitis: Comparing Upadacitinib to Tofacitinib and Rescue to Sequential Salvage therapy.

JAK inhibitors were effective for ASUC with a reassuring safety profile. Upadacitinib use resulted in a significantly lower rate of colectomy by week 52, with higher rates of clinical remission, CFCR, and biochemical remission during follow-up. Sequential salvage therapy may represent a viable therapeutic option for patients otherwise facing colectomy.

Diagnostic performance of the Japan Narrow-Band Imaging Expert Team and pit pattern classifications for small polyps using next-generation endoscopy.

JNET showed similar observed diagnostic performance across expertise levels, whereas pit pattern classification showed greater between-group differences in sensitivity and negative predictive value. These findings suggest that JNET may serve as a practical framework for routine optical diagnosis of small lesions.

Faecalibacterium prausnitzii EXL01 Strain for the prevention of multiple-recurrent Clostridioides difficile Infection.

A single, well-characterized bacterial strain was associated with restoration of key microbiome functions and low recurrence rates in high-risk rCDI. High rates of transition to immune active disease in a cohort of strictly defined immune tolerant patients with chronic hepatitis B: results from the RADICAL consortium.

The majority of this large cohort of well-defined IT patients transition to IA disease within 5 years of follow-up, with the highest risk observed in those with a high-normal ALT, suggesting that these patients may benefit from either more frequent monitoring or pre-emptive therapy.

Hospitalisation in cirrhosis is linked to distinct gut microbiome structural and functional profiles: a multinational metagenomic study.

Gut microbiome composition and function are associated with short-term hospitalisation risk and provide additional prognostic information beyond clinical variables in a multicountry cohort. Prior endoscopic surveillance and diagnosis of Barrett-related dysplasia and cancer: evidence from a nationwide Dutch cohort.

In this nationwide cohort, 90% of patients with Barrett-related dysplasia and cancer and 65% undergoing organ-preserving endoscopic treatment had not undergone prior endoscopy. Hepatitis B virus proteome analysis identifies apolipoprotein C1 facilitating particle production and virus entry.

We established a method for obtaining highly purified HBV virions and fSVPs and identified the HDL component ApoC1 to associate with both particle types. ApoC1 promotes HBV and HDV infection most likely via SR-B1 and facilitates viral entry.

Lactulose with or without TeleTai-Chi for the prevention of falls: A sequential multiple assignment randomized controlled trial (LiveSMART).

In patients with cirrhosis and portal hypertension without prior overt HE, lactulose reduced falls and fall-related harms. Sequential lactulose followed by TeleTai-Chi was also superior to enhanced usual care.

Safety and efficacy of zetomipzomib in relapsed or insufficiently responding autoimmune hepatitis: Results from the randomized, double-blind, placebo-controlled, phase 2a PORTOLA study and open-label extension.

Zetomipzomib resulted in numerically higher CR rates than placebo, with evidence of steroid-sparing activity in a difficult-to-treat population, and warrants evaluation in larger randomized studies.

Restoration of aged liver regeneration by youthful systemic factors via LTβ-driven hepatocyte reprogramming.

Young B-cell-derived LTβ promotes aged liver regeneration by promoting the transient somatic reprogramming of periportal hepatocytes via LTβ/LTBR signaling. Our study highlights the therapeutic potential of LTβ-based strategies to increase regeneration in aging populations. Impact and implications The regenerative capacity of the liver decreases with age, limiting the utilization of elderly grafts. This study demonstrates that youthful systemic circulation rejuvenates aged liver regeneration through young B-cell-derived lymphotoxin β (LTβ), which induces the transient somatic reprogramming of periportal hepatocytes via LTβ/LTBR signaling. These findings are highly relevant to transplant surgeons, hepatologists, and aging researchers. Practically, short-term pharmacological LTBR agonism could be utilized by clinicians to therapeutically "pre-condition" elderly donor livers or enhance post-hepatectomy regeneration in older patients. Furthermore, our work establishes heterochronic transplantation as a powerful clinical research platform to investigate anti-aging mechanisms and rejuvenation strategies across solid organs.
