By implementing coordinated strategies, including patient navigation, digital tools, and open access colonoscopy, health care organizations can close critical gaps in the CRC screening continuum and decrease CRC incidence and mortality.

Hypervigilance and symptom-specific anxiety can overlap with al the phenotypes, and E-DGBI can coexist with conclusive GERD. Therefore, there are intricate interrelationships between Montreal, Lyon and Rome criteria that aid evaluation of troublesome symptoms, and concepts from each of these contribute to accurate phenotyping and optimal management of patients.

This review highlights the evolving role of P-CABs in gastroesophageal reflux disease and FD and emphasizes the need for further research to refine their clinical positioning, optimize therapeutic strategies, and evaluate long-term safety outcomes.

A spiral basket configuration improved early procedural efficiency and reduced device escalation and device-related costs, without improving the final stone-clearance rate or increasing adverse events. The spiral basket was not compared with balloon extraction and should not be interpreted as superior to it. Ineffective Esophageal Motility and Its Predictors in Gastroesophageal Reflux Disease Patients with Pathologic Acid Exposure Time.

Machine Learning Clustering Identifies Achalasia-Spectrum Phenotypes in Distal Esophageal Spasm.

Machine learning clustering successfully identified a DES phenotype characterized by high premature contraction frequency that clusters with type III achalasia. Morphological, Molecular and Motility Changes in Human Small Intestine After Loop Ileostomy.

Although morphological, molecular and motility changes including down-regulation of contractile protein expression were observed in the distal loop, number of PDGFRα + cells, ICCs, and neuronal cells were preserved. These results may explain restoration of the bowel motility after ileostomy take-down with low incidence of ileus.

IBS-C and IBS-D showed similar symptom relief after FODMAP restriction. Notably, 44.9% of IBS-C patients shifted to normal stool consistency (IBS-U), with greater symptom relief and reduced pain. FODMAP restriction does not necessarily exacerbate constipation in IBS-C and remains a viable treatment option in real world clinical practice.

Review Article: Accelerating Coeliac Disease Clinical Trials-Beyond Celiac Meeting With Industry, Academics, Patients and the FDA.

addressed the ideal length of the double-blind period, including a shorter 24-week double-blind study with an open-label extension versus a 52-week double-blind design. Trial duration should balance scientific rigour, patient retention, and the need for longer-term safety and durability data, with flexibility to tailor the approach to context. Conclusions Progress in coeliac disease drug development will require patient-centred, scientifically robust but feasible trial designs with appropriate endpoints, justified use of gluten exposure, consideration of improved biomarkers to reduce patient burden and early regulatory engagement. Continued multi-stakeholder collaboration, including dedicated consideration of paediatric drug development, is essential to advance effective therapies.

A framework and shared vocabulary to identify actionable protein biomarkers and enable optimal precision medicine: Myth-Busting with Unicorn Biomarkers.

We propose that minimum technical and biological standards must be met before a molecular measurement can be designated a "biomarker", beyond minimal criteria of association between biological signal and disease state. By raising the bar with Unicorn Biomarkers, this framework aims to guide researchers, regulators, and industry stakeholders in identifying actionable biomarkers and advancing precision medicine.

Fibrosis regression in chronic liver disease: the pathologist's view.

The clinical significance of regressive features is hitherto unstudied. In this brief review, we are addressing shortcomings in (i) the histological assessment of fibrosis regression across the whole spectrum of common chronic liver diseases and (ii) identification of a point of no return in advanced fibrosis and argue that the hepatopathologists' community should rise to such challenges, perhaps hastened by development of artificial intelligence solutions applied to liver biopsies.

Advances in Understanding Calcium Sensitization in Gastrointestinal Smooth Muscles: Pathways, Interstitial Cells, and Disease.

This review updates our current understanding of the regulatory mechanisms of calcium sensitization with the recent findings from genetically engineered mice with deficiencies in protein kinase C and ROCK2 calcium sensitization pathways, and the involvement of interstitial cells in regulating GI smooth muscle cell excitability. Dysregulated calcium sensitization pathways in motility disorders are summarized, to potentially aid in the development of new pharmacological strategies to prevent and treat GI motility disorders.
