Diagnostic Impact of Liver Biopsy Among Antinuclear Antibody Positive Individuals With Mild Liver Enzyme Elevation.

RNA binding protein CAPRIN1 suppresses STAT1 translation and interferon signalling to promote HBV replication.

CAPRIN1 is elevated in IFN non-responders and further upregulated during HBV infection/replication. By facilitating stress granule formation and stabilising STAT1 rG4 structure, CAPRIN1 blocks ribosomal scanning and suppresses STAT1 translation. OCE-205, a selective V1a receptor mixed agonist-antagonist, for the treatment of hepatorenal syndrome-acute kidney injury: A phase 2 randomized trial.

OCE-205 was well tolerated, with no evidence of excessive vasoconstriction or related events of ischemia or respiratory failure at any dose level. OCE-205 had a predictable, capped maximal efficacy that improved HRS-AKI in numerically more patients than placebo, though was underpowered for statistical significance. OCE-205 warrants additional investigation as a novel HRS therapy with a favorable benefit/risk profile.

Chromatin remodeling in pericentral hepatocytes modulates MASH through CYP450 activity.

DPF2 in Lgr5 + hepatocytes is a critical regulator of hepatic metabolism, acting via the CYP2-atRA-AMPK axis. These findings reveal the mechanistic basis for zone-specific metabolic control in the liver and identify atRA homeostasis as a potential therapeutic target for MASLD and MASH. Impact and implications This study dissects chromatin regulation across defined hepatocyte populations in distinct liver zonations and demonstrates that DPF2 activity within a rare Lgr5 + hepatocyte subset is sufficient to drive metabolic reprogramming and MASLD progression. These findings are particularly important for liver researchers and clinicians, as they explain why liver-wide genetic manipulations or bulk omics analyses can obscure key disease-driving mechanisms confined to spatially restricted cell populations. From a translational perspective, our data suggest that targeting chromatin regulators or retinoid metabolic pathways may provide effective and safe therapeutic benefit; notably, systemic atRA administration can still confer robust protection or prevention at the whole-liver level. Although these implications are supported by rigorous mouse genetics, high-resolution transcriptomic analyses, and validation in human liver tissues, clinical translation will require confirmation in larger human liver cohorts and carefully designed patient studies.

Long-term outcomes after nucleos(t)ide analogue cessation in chronic hepatitis B - follow-up from the RETRACT-B cohort.

At 10 years, 17% of patients achieved off-treatment HBsAg loss, whereas more than half has been retreated. Patients with HBsAg levels <100 IU/mL benefit the most from NA cessation with a HBsAg loss rate of 52%. Based on our data, finite NA therapy is not recommended if HBsAg ≥1,000 IU/mL or in cirrhotic patients. Impact and implications Based on our study, we can conclude that finite NA therapy can be considered in patients with HBsAg levels <100 IU/mL when aiming for HBsAg loss, since they have the highest likelihood of achieving this endpoint (52%) and lowest need for retreatment (28%) during long-term follow-up, but only when strict monitoring can be guaranteed. In contrast, finite therapy is not recommended in patients with HBsAg levels ≥1,000 IU/mL or those with a history cirrhosis, especially as cirrhosis is a known risk factor for hepatic decompensation and HCC. In addition, our data can be used to further improve patient monitoring after finite NA therapy, especially in those who successfully remain off-therapy for five to ten years.
